Multiethnic Families: Registration General Information REGISTRATION FEE: $200 (includes luncheons, refreshments and conference materials. Fee is $200 for all three days or $80 for one day). Make check payable to: Mail with the registration form to: DEADLINE: Deadline for advance registrations is January 15, 2006 postmarked by January 3, 2006. Form of payment must accompany the registration. Credit card numbers submitted with registration will be processed upon receipt. Registrations will not be processed without payment. Government and company purchase orders with authorized signature will be accepted and must accompany the registration form. Participants registering by purchase order will be billed for nonattendance unless notification of withdrawal is made by February 16, 2005. REFUNDS: Requests for refunds will be received at the UH Conference Center by February 16, 2005 in writing. No refunds will be made thereafter. Refunds will be mailed. Please allow approximately three to five weeks for processing. ACCESSIBILITY ASSISTANCE: If you would like assistance due to a mobility, hearing, or sight impairment, you are warmly encouraged to contact the Conference Center at 956-8204 by December 21, 2005. HOTEL ACCOMMODATIONS: There are a limited number of rooms available at the Ala Moana Hotel. Please contact Yvonne Yamashita (contact information below) for further information. PARKING: A pass is available at the Imin Center Conference Hall for $4 per day. Please see registration form for details.
